How to Submit your Samples:
For your convenience, here is the step-by-step process for submitting samples
Step 1:
Begin by filling out the W-9 form for billing information. Fax to (312) 850-3303 or Email to laick@metrotechlab.com
Step 2:
Along with your completed W-9 form , please provide Phone and Email address of your main contact for the asbestos and or lead testing process. Include any Special Instructions.
Step 3:
Samples must be adequately contained to prevent exposure to laboratory personnel. Samples should be placed in a sealable plastic bag or centrifuge tube. Samples that are not contained will not be accepted. Each sample must be uniquely identified and submitted with a Chain of Custody (COC). The required information to be included in each COC include: turnaround time (TAT), sample identification numbers, name of the company or individual who will be invoiced for analysis and the contact information such as address, phone number and email address where both invoices and analytical results can be sent.
Please note: If you are in need of immediate turnaround, we ask that you please call the laboratory ahead of time to schedule analysis.
Immediate turnaround time for PLM and PCM analysis reporting for samples submitted before 3 pm will be no less than 2-4 hours from the time the samples were submitted.
Immediate turnaround time for lead analysis reporting for samples submitted before 3 pm will be no less than 4 hours from the time the samples were submitted to the lab.
All samples requiring immediate turnaround submitted to the lab after 3 pm (unless prior arrangements have been made), will be analyzed the morning of the next work day and analytical results will be available after 9:00 am.
Once your samples are ready, please fill out the proper Chain of Custody (COC) form:
- COC form for Bulk PLM samples
- COC form for Asbestos PCM samples
- COC form for Lead Air and Wipe samples

Step 4:
Drop off your samples at our lab. If lab is closed, drop off samples in our drop box, which is located outside, on our front door.
MTL now has a 15-minute standing zone right outside our front door for clients dropping off samples!
